What puzzles me is if my e-mail address was selected as a winner, how come I need to fill out the information including my e-mail address??? I'll need to clarify that with them when I collect my winnings (which they have made extremely easy - I just gave them my bank account info and they are going to wire the funds directly into my account!)

It's there as a way to verify that you are who you say you are. For example, if you posted the winning notice on a message board, any number of people (especially lurkers! [:0]) could write back to the lotter officials and claim to be you. Your email address just as important than your social security number. Don't give it out to just anyone.
